Report this as  additional income. Additional income is money that wasn’t reported on a 1099-MISC or 1099-G. This is for income paid by check or cash, money from a barter or exchange or  income reported on a 1099-K.

Report this as  additional income. Additional income is money that wasn’t reported on a 1099-MISC or 1099-G. This is for income paid by check or cash, money from a barter or exchange or  income reported on a 1099-K.

Will I be able to deduct expenses (mileage and parking) or will it not be worth deducting anything if the amount is small? (A couple thousand)

You could deduct mileage if you went from your office or office in your home to the person's home. Or from another job to that person. You can't deduct that mileage if it's commuting (driving back and forth from your home to the patient).
