@Critter-3 Wow---it is great that you saw this and know how read user's minds to figure out it is a "state thing."
First I live in FL and know of these things ... and google searched it and it seems only FL has this but I could be wrong on the state ... will see if OP writes back. I love Google ... I can usually find what I am looking for in 20 second or less ... many of the posters on this site should use it before they post to get their answer.
still not really sure what you did. however, if you filed as self-employed you are not a corporation unless you incorporated and filed as self-employed which would be a big mess. first, you would need to prepare a corp return if it is taxed as a C-Corp there's nothing to report on you 1040 except salary from it. however, treating payments from it to you as SE income can cause substantial tax problems. If it's taxed as an S-Corp there would be a K-1 which would need to be included on your return. The same is as to salary would also exist. One other possibility is you elected to be a single-member LLC which also would require registration with Florida. however, if yo a service that did the filings for you. contact them
