Do I actually enter the word Form and Line on a Schedule V?

I amended my federal return and am now amending my Montana state return. The instructions literally say: Line or Box must be entered.

Do I actually enter the word Form and Line on a Schedule V?

Yes. If you changed the Gross Revenue amount on Schedule C of your Federal return you would type "Federal Schedule C" in column 1. and "Line 1" in column 2.
