You won't get a Form 1095-C for a Medicare supplemental policy.

Form 1095-C is only issued for an Employer-Provided Health Insurance Offer and Coverage. The Medicare supplemental policy is one you purchase yourself to cover medical costs that are not covered by Medicare Parts A and B.
