Not sure what tax form you are asking about.  You need to have the documents that report your income such as your W-2's and/or 1099's received in order to file a tax return for 2019.  Everyone files a tax return on a Form 1040.

Do you mean you have a W-2?  Then use it to prepare your tax return.  Since you do not qualify for a prepaid Visa, choose a different method to receive your refund if you are getting one.  You can have direct deposit to a bank account or you can have them mail a check to you.
