If you amended the 2016 tax return using the desktop amend software the address on where to send would be shown on the Filing Instructions page.

Otherwise, go to this IRS website and click on your state for the address on where to mail the Form 1040X - https://www.irs.gov/filing/where-to-file-paper-tax-returns-with-or-without-a-payment
