When mailing my 1040x, do I need to redo the original 1040 form or only mail the papers which include the revisions?

I printed my amended forms using TurboTax and it included blank 1040 forms, just wondering if I have to fill those out or only mail the papers that are already corrected.

You only send the form with the revisions which is the 1040x.
