It would not be a bad idea to create an amended return and include the Form 1099-R.

Even though it may not affect your bottom line refund or balance due, the amount reported in box 1 of the Form 1099-R should be reported on your return as a distribution.  The IRS will have a copy of that form and will expect to see it reported.

Does Box 2a of the 1099-R  actually have a zero in it, or is it empty?
