Received a 1099B 2 weeks after filing taxes. Sale was a wash, sold for what I paid. This was a tax free bond (state owned utility). Do I need to file amended return.

You should amend your return. If the IRS does not have the basis amount, they will tax you on the sale and assume zero basis.
