Do i need my slip to add to my quarterly payements or can I just send a check with my ss # and form 1040-ES?

You can either mail a check with the 1040-es   OR   pay online directly :  https://www.irs.gov/payments/direct-pay

I don't know what you mean by your "slip." The Form 1040-ES Payment Voucher (not the entire Form 1040-ES) is what you have to send if you mail a check. If you pay online with IRS Direct Pay, as Critter suggested, you do not need any form or slip.
