My baby born in 2022 was in the NICU and we have large medical bills because of it. Can i deduct it if we haven't paid anything on it yet?

Sorry.  No.   Only medical bills you have PAID can be entered on a tax return.  You cannot enter bills that you have not yet paid.

You can only deduct medical expenses that you have paid with out of pocket funds and which were not reimbursed.  Since you mad no payments in 2022 you cannot claim any medical expenses on the 2022 tax return.

If you paid for those expenses in 2023 they can be entered on you 2023 tax return you file next year.
