If a possible Qualifying Relative is on Medicaid, should that be figured into the support test calculations? If so, where would you get the numbers?

Medicaid is not support provided by the individual and would not be calculated in the amount of support provided.

See this IRS website for how to calculate support to an individual - https://apps.irs.gov/app/vita/content/globalmedia/teacher/worksheet_for_determining_support_4012.pdf

It's not on the worksheet, but logically, shouldn't health insurance costs be included in the 'total' cost of all support?

It's not provided by the individual, but benefits from the state like SNAP would be entered on line 23, right? Medical expenses reimbursed by insurance are specifically excluded from expenses, but it would make sense for Medicaid to be treated like other benefits. This page specifically includes Medicaid among benefits considered support provided by the state but doesn't say what that means in practice. - <a rel="nofollow" target="_blank" href="https://apps.irs.gov/app/vita/content/06/06_04_075.jsp?level=basic">https://apps.irs.gov/app/vita/co...>

Since there are no costs to the individual associated with receiving the Medicaid state supplied benefits, there would be no way to include a dollar amount of support being provided by the individual for the Medicaid benefit received.
