It's titled: 2020 Federal Grantor Information. And the body of the document it says: The following income is to be reported on the income tax return of the above-named grantor.

The trust tax ID: 47-6891537

The Trust Company is IRA Services Trust Company

There is no Form # on the document.

It would probably be reported under Miscellaneous Income, Other Reportable Income.
