That 1098-G shows that you received a refund for 2014 and 2015 in 2016.
If you did not, then I would contact the state of Arizona Department of Revenue.
If you did receive both those refunds, then you would need to report them both IF you itemized your deductions in 2014 or 2015 (meaning you would have had a Schedule A to go with your Form 1040 in those years).
You can do that by adding another 1099-G. If you already filed and itemized deductions in those prior years, then you will need to file an amendment for that second 1098-G.
