I received a letter from a previous employer that said I wouldn't be receiving a W2 for a repayment I made in 2014. How do i reflect this on my amended taxes for 2015?

If you made the repayment in 2014, you need to amend 2014, not 2015.  The following refers to 2015, but is equally valid for 2014:

Be sure to keep the letter from your employer (and documentation of your repayment) in case of an IRS query.
