If you made the repayment in 2014, you need to amend 2014, not 2015.  The following refers to 2015, but is equally valid for 2014:

If you made the repayment in 2014, you need to amend 2014, not 2015.  The following refers to 2015, but is equally valid for 2014:

Be sure to keep the letter from your employer (and documentation of your repayment) in case of an IRS query.
