I learned that a form needs to be filed if I had HSA in 2016, is that included with my Federal returns or would I need to file it separately?

That form would be included in your return and calculated based on the HSA information you entered when you entered your W-2.

That form would be Form 8889, Health Savings Accounts and should print when you print your return.
