I have multiple doctors. I would like to make notes that to give a total but explain how I came to that total. How do I do it? Thanks.

Unfortunately, there isn't a place in the 1040 tax return for notes. 

If you are filing my mail, you can include the statement in addition to your return, but there is no way to attach notes in an e-filed tax return.

I advise you to keep your notes with a copy of your tax return in case there are questions later from the IRS.
