Look at your tax return. You have not mentioned what tax year the letter is for--so read the letter carefully and make sure you look at the return for the year they are asking about. There will not be an image of the 1099R in the pdf, but the worksheets will show what you entered.

1099-R's are issued for all types of retirement accounts and they are reported differently. Where is is found on your tax return depends on the box 7 code, the IRA/SEP/SIMPLE box and what you did with the money - rolled it over, converted it, kept it, etc. Most 1099-R reporting will be on the 1040 line 4(a,b,c or d) depending on the codes and year. Some 1099-R's do not go on line 4 at all, depending again on the codes.
