No..not for that. You could just get the 1040ES forms on a PDF document from the IRS and write in your own amounts and mail them in with a check.

There's also a way to arrange direct Debits without any forms using the following, but I've never used it, and in either case, you need to keep a good record of what you paid each quarter. pay attention to the Due-by dates because the "quarters" are not equal:
