@Chasey-Tezak before I completely reply did you work for a NYS company and assigned to a NYS office? What type of work do you do for them?
Your employer though, if there was a mistake, should be the one filing and reimbursing you for both. you should not have to file a return.

I agree with maglib. While this quote is not authoritative (it's from an insurance company that offers PFL services and benefits), they say, "If an employer collects payroll deductions for PFL and for any reason does not offer a PFL policy, the employer must return those funds to employees." See, The Standard.website under "Returning Employee Copntributions".
