Hello I set up a payment plan on a debt last March . It was 1600 then . It was approved And all set to come out my account 40.00 every month . IRS never took it out. I assumed it was because of COVID

You would have to contact the IRS regarding your payment plan.  You can call the IRS at 800-829-1040 (individuals) or 800-829-4933 (businesses)
