TurboTax does not show form 8962; i am using Turbotax 2016; the form did not print now the IRS is asking for same. thank you for your help.

The 8962 is populated after you enter the form 1095-A in the Health Insurance tab ... make sure you have updated the program so all the forms are present and working.
The Form 8962 will be included with the tax return if a Form 1095-A that you received for Marketplace Insurance was entered in the Health Insurance section of the TurboTax program.

Form 8962, Premium Tax Credit
Copy of your Form 1095-A, Health Insurance Marketplace Statement
A copy of the IRS letter that you received.
Only if the refund or owed amount has changed: Also include your Form 1040
There wasn't a 1095-A, I have a 1095-B. I had to enter the months that i did not have health insurance.

A Form 1095-B is not entered on a tax return. If you did not receive a Form 1095-A and never had health insurance through the Marketplace then there would be no reason to have a Form 8962.
Contact the IRS if you did not have health insurance through the Marketplace during 2018 as they probably have incorrect information concerning your insurance.
Then you would need Form 8965 in your tax return, not Form 8962. Form 8965 calculates the penalty for the months that you were not insured. Form 8962 is only for those who qualify for Premium Tax Credits because they have insurance from a Health Insurance Marketplace and therefore receive a Form 1095-A. It seems to me that the IRS would only be asking for Form 8962 if they received a copy of Form 1095-A, which seems unlikely if you received a Form 1095-B (unless you should have received both because you changed to or from a marketplace plan).
