How do I need to prove gambling losses? I have played exclusively with my players card at only one chain. Would an activity report by sufficient proof?

I have won several jackpots but I have also lost as well.  I believe that I will be about even after everything.

Yes, an activity report is adequate. You do not have to provide proof at the time you file your tax return; only if you are audited. Gambling losses are only deducted as an itemized deduction on schedule A. You must report your full winnings as income on line 21 of form 1040.

For state tax purposes, a few states do not allow a deduction for losses.
