I amended my 2015 Fed tax return and received a refund chk in 2017. I would assume that I need to claim this as income in 2017. Please advise how I do that (forms?).

Federal tax refunds received for prior year returns are not taxable as income, just as federal tax payments made in prior years are not deductible.
