Can I combine my current 2017 tax return with an IRS tax notice (from a prior year) and repay both under Form 9465?

Yes, you should complete an new Form 9465, if you did not include the balance from your previous agreement with your new request. There will only be one installment agreement which includes all of the tax years that you owe. 

You can try using the online tool or you can complete, sign and print your form to mail it to the IRS.
