Notice CP 53 and State of Michigan change

Hoping someone can give me a hand here with this situation. I filed my grandma's taxes for her. She owed ~$12K in federal and received ~$1.1K refund in state. Both returns were filed April 13th and accepted shortly after filing as normal.

 

My grandma recently received Notice CP 53 from the IRS (federal) stating that her refund could not be deposited because her account number wasn't provided on her return. It was provided - the ~$12K was withdrawn from her account as expected. 

 

We then received a notice from the state that she OWES ~$600. She has not yet received her ~$1.1K refund from the state of Michigan.

 

Is it possible that something changed on the return after it was accepted by the IRS and State of Michigan? I am not a POA (Form 2848), so I have not been able to discuss this with the IRS/Michigan and my grandma is not capable of having the conversation. I have requested a transcript from the IRS but not sure how long that will take to receive. 

 

Does anyone have any advice or ideas what happened?

 

PS - I file ~10 returns a year, so it is unlikely that I have a mistake of this magnitude.

    Sounds like the IRS letter is an error and since you have proof that her federal liability has been settled I wouldn’t do anything. Regarding the state, she should receive a letter explaining the adjustment so wait a few weeks.
