If you are mailing federal return - attach any forms to the return that have federal tax withheld. And if you are mailing your state as well, please attach any forms that have state tax withheld. You don't have to mail other forms that don't have the respective tax withheld, such as mortgage interest form 1098 or interest form 1099-INT.
