forgot 1099-B

  I forgot to file  a 1099-B in my tax return( loss over $60,000)

I suppose I should fill out a 1040X 

In filling out 1040X, everything in column A will be the same in column C ?

Not getting any additional tax refund( already carried forward $3000 from previous tax losses.)

forgot 1099-B

Yes, you are correct that there will be no change if you already had prior $3,000 carryover losses. But you must file to show the $60,000 loss and the carryover losses. Also, the IRS has a copy of the 1099-B and will be looking for the gross proceeds and basis from it on your tax return.
