Took an early pension withdrawl 1099-R and I understand the penalty, Issue is the 1099-b worksheet?

I have imported the information from the 1099-R from my company pension withdrawal. I realize there is a penalty. My issue is when doing the federal review there asking for :cost and other basis, sales price, date of sales and exchange and date sold. I didn't sell stock this was a company pension and this information was not provided to me. Help?

Enter your 1099-R in the Retirement Plans and Social Security section, not the investment section.

Sign into your return and select the 'take me to my return' button
Type '1099-r' in the search box then select the 'jump to' link.
