Question about Form W-2, "Additional wage and tax information" Part 4, Question #6

I am having trouble filling out Form W-2, "Additional wage and tax information".

I don't understand what should put down for question #6 in part 4, Form W-2.

Part 4, Question #6 says: "Enter any amounts forfeited from a flexible spending account."

The following are details you might need to help me solve this problem:
Our plan year went from July 2015 thru June 2016. The employer allows a roll over into the next year. We chose to have your employer set aside $5,000 for the 2015-2016 plan.

In 2015, My wife's W-2, line 10 had an amount of $1,666.64.
In 2015, we took $1,400 out of our FSA.

In 2016, My wife's W-2, line 10 had an amount of $3,333.36.
In 2016, we took $3600 out of our FSA.

Can anyone help me?

It's not clear to me if you are an employer filling out a W-2 for an employee, or maybe you are an employee filing out a W-4 for your employer and mistakenly called it a W-2. In what program if any, are you being asked these questions? TurboTax Business maybe? Or TurboTax Home & Business? Looking that the W-2 and W-4, neither is divided into parts, therefore there is no Part IV on those forms.

It's from Turbo Tax. I'm pretty sure it's a worksheet. 

I found it on Form W-2      "Wage and Tax Statement"

On page two:   "Additional Wage and Tax Information"

Part IV     Dependent Care Benefits

6.  Enter any amounts forfeited from a flexible spending account ________. 

Any clarification you can provide would be greatly appreciated.
